ASSUMPTION OF RISK, RELEASE AND HOLD HARMLESS AGREEMENT AND PHOTO CONSENT
I am voluntarily participating in the activity described below (“the Walk”). I acknowledge that this activity does not come without the risk of injury or harm. Nonetheless, I agree to assume all risk and liability, and to assume full responsibility for all costs, including, but not limited to medical, dental, and rescue, that may be incurred while participating in the activity, including travel to and from this activity or any other event incidental to the activity. By signing below, I completely release and discharge and agree to indemnify, defend and hold harmless Beyond Fundraising Inc., and their directors, officers, staff, agents, volunteers, consultants and contractors (the “Released Parties”), including Global Impact Tours Inc. dba Global Impact Productions, the Toronto Department of Transportation Services and the jurisdictions through which I will walk, from any and all causes of action, claims, rights or demands, for any and all loss, injury, death or damage arising out of or related to my involvement in the activity, travel to or from the activity, or any event incidental to the activity, whether or not caused by the negligence of the Released Parties. If the Released Parties incur any of these types of expenses, I agree to reimburse them.
I am physically capable of participating in the Walk, and my medical care provider has approved my participation. If I am aware of or under treatment for any physical infirmity, ailment or illness, my medical care provider knows of and has approved my participation in the Walk. If I have been diagnosed with any psychological conditions, my mental health provider knows of and has approved my participation in The Down There Affair. I acknowledge that I, and I alone, am solely responsible for my personal health and safety, the condition of the gear I will use on the Walk, and any other personal property I bring with me. I will obey the rules for participation in the Walk and I will abide by all rules and regulations established by the Walk organizers and personnel as well as the transportation laws of all jurisdictions in which I will Walk.
This agreement is binding on myself, my heirs, executors, administrators, assigns, and successors in interest, and applies to all losses, whether known or unknown, foreseen or unforeseen, related to my participation in the activity.
Also in consideration for my acceptance as a participant in the activity, I grant full permission for the Beyond Fundraising Inc. and Global Impact Productions to use photographs, portraits, film and videos of me and quotations made by me about the event now and in the future in any manner or media and for any purpose that supports their mission, including, but not limited to materials used for educational, promotional, advertising, or fundraising purposes, and to use my name. I decline payment or other consideration for granting these rights.

Description of Activity: The Down There Affair will be a 7 km and 12 km walk in Toronto, ON (final route subject to change) with an on-site festival.
THE UNDERSIGNED STATES THAT SHE/HE IS 18 YEARS OF AGE OR OLDER, THAT SHE/HE HAS READ THIS AGREEMENT; AND THAT SHE/HE FULLY AND COMPLETELY UNDERSTANDS THE TERMS AND CONDITIONS AND AGREES TO BE BOUND BY THEM.
If Participant is under 18 years of age:
I, as parent or guardian of the participant named during the registration process, hereby give my permission for my child or ward to participate in the above-described activity and agree individually and on behalf of my child or ward, to the terms of this Assumption of Risk, Release and Hold Harmless Agreement and Photo Consent.
